1. You filed the Orange Form to opt out of the class action by March 30, 2000, and have proof of your Fen Phen usage in the form of pharmacy records, medical records, or actual pills in your possession; OR
2. You have moderate to severe pulmonary hypertension diagnosed by an echocardiogram; OR
3. You require heart valve surgery to repair or replace one or more heart valves; OR
4. You have already had heart valve surgery to repair or replace one or more heart valves.
